TV sarcasm and medical mysteries

There are two great things about "House": The main character's attitude and the ingenuity of every episode's medical mystery. Sarcasm is something we can relate to well in terms of TV viewing. I know someone who displays the same sarcasm (it's really true!), and I tell you it's really an irritating attitude to deal with (as in you wanna kill him)! It's fun to see on TV though. What REALLY amazes me is how the writers could come up with such medical mysteries! They must have really invested on such ideas!
