Perhaps one of these sites will have a form you find suitable: http://www.findlegalforms.com/xcart/customer/home.php?cat=563 http://www.myconfidentiality.com/ Technically these are not free sites, but usually they will offer samples of many of the forms. I've used findlegalforms quite often.
