small vessel disease in brain matter

I had an MRI done of my brain and I apparantly have advanced small vessel disease. I am not exactly sure what all this means and what I can do to stop it. I am 53 years old, female, weighing 146 lbs. at 5'6. I know I need to lose around 15 pounds. I do play tennis 3 or more times a week for exercise.
