you're scheduled for a vital surgery. you are rolled in the ER, and the surgeon and his team prepares for your surgery. you're given a shot of anaesthesia(spelled right?). surgeon cuts you open and starts the operation. you're a bit awake but can't really feel anything anymore. and all of a sudden, the surgeon speaks, and you hear those words (see pic)..
