My two favorite movie twist, that caugth me completely of guard, were in Fight Club, and the Usual Suspects, in both cases were masterful finishing touches to already great movies.Also in Oldboy the final revelation is a great jaw dropper.

THE USUAL SUSPECTS is the first movie that comes to mind when I think of "twists". I watched it when I was younger and absolutly loved that ending.

I know a lot of people will say 'Sixth Sense,' but my favorite movies with a twist are 'The Village' and 'Campfire Tales.' I never saw either twist coming.
