What happens after death ?

I really wonder some times that wat happens after someone dies ....does he forget all wat he had done ih his life and takes a rebirth....??? How about our own consciense which cannot die...it remains with us forever....wat happens to that...do we leave our bodies ans our souls still exist and remember everything...??? but i have also heard that the same souls r reborn again with a new body...then wat happens to our consciense ?? if u hav any answer do tell me....
