Questionnaire on origin that comes with every form we fill out these days

I am interested to find out how many people out there. feel a way about filling out this form. The one that asks us what colour we are, eg. Are you White British, Black British?
Why do we need to give this information? What is this information used for?
Do you think it would be fairer if this form was not used at all?
