Star Rating Script

Does anyone know of a script or some sort of application to add to my website for rating purposes. I am looking for a simple 5 star rating system. Something where a user can just click a rating on a scale of 1-5 or 1-10 and it automatically registers with only 1 vote per user. Any suggestions?
