Accidents are always a series of mistakes.

I watched almost every episode of the program 'Seconds from Disaster' on Nat-Geo and really appreciate the way the program has been framed. It not only contained all the necessary information about the disaster but also involves the real time observers/survivors who narrate the horrifying story about the accident. However, the saying 'Accidents are always a series of mistakes.' does not fit in all circumstances. All errors are not always human. What you say?

all errors are not always human, what else? who makes error? if it is not human. all accidents are not always mistakes it could be carelessness or over happiness. error is an error whether human or non human. but accidents are products of over excitement or carelessness. all disasters could put to control if we exercise little patience, caution and carefulness.
