Think depend on what level was the relationship between you and him/her. If know very well and was a very good friend then can simply ask them did he/she have any defective teeth? or indirectly let them know the problems by telling them you yourself last time have a really bad breath due to the teeth problems and how embarasing it cause you but got to be very sure you know how to convey it by the most proper way.

it's kinda hard to tell them straight away. because it will really offend them. and honestly, i don't know how to tell them straight about it.

Its an awkward situation, as you may not be able to tell your boss that "your mouth stinks man !!" or your body builder neighbour the same thing, neither can you to a beutiful lady whom you adore like anything. It a natural concept that if you eat something and don't wash your mouth then it will get deposited on the crown of your teeth which is coated with flouride and it will start rotting it. Then, it will start producing some bacteria on that as usual and which causes smell. The best way to tell them is give them bad smelling plant as a gift and write beautifully that would you like to be counted in the same category? It will kill two birds with a stone.

I would be respectful about of course, you could always ask them if they would like a piece of gum or mint. If it was someone that you knew pretty well, you could always kindly say something to them. Don't be rude about it!

tell them in a nice way that you have noticed they sometimes have bad breath. Most people would appriciate the truth. Bad breath can be embarasing. I would want to know.

Everybody will have bad breath. It is better to clean the teeth twice a day. Dentists say: whenever you see a mirror, you comb your hair. Likewise whenever you see washbasin, wash your mouth. Artififical chemical coated mouthfreshers are not good for health.
