We just had a moment of laughter in the office. Maybe it's not that funny, but we thought that it was. I work in a broker's office and one of the girls was busy completing an application form for life benefits. One of the questions on the form is: Have you ever committed suicide? The next question: If yes, please submit details. Well...

really? i have not encountered such a question in my 6 years of filling up forms, application forms, etc from either of the companies i did applied for, even in insurance, medical, etc...:) thank you for sharing.. it is quite a good one. he he

That question is really weird. It's my first time to encounter such query in an application form. It's really funny. The company is really making sure that their members are guided. Smile.
