It is a common courtesy to respose to some one who is trying to reach you. So that the next level of communication will start.

I think this started as a practice may be in India from England and also similar words were prpopgated in the rest of the world. This seemingly has over the years become a standard practice .

Hello is a word used to wish a person. when we see any one we wish that person first and then start talking. the same way when we hear some ones voice we wish him first and then continue. if you want you can use any other word which used to wish a person.

hi... it is a norm already... we always use either "hi" or "hello"... other ways could be simply saying "good morning, good afternoon and goodevening". it is simple courtesy. courtesy goes a long mile... good day

Because you say good bye when you hang up. It wouldn't make any sense to pick up the phone and say good bye. It would be fun to try though.
