Approaching a threesome

I just recently found out a few of my friends have engaged in threesomes. I was just curious, if you wanted one with a close friend and your lover, how would you ask your friend? It seems like it would be really uncomfortable if your close friend said no! Would you be more likely to ask if it wasn't a close friend, but just a normal friend?
On the other side, how would you begin to tell your lover that you wanted a threesome? What if your lover said no? What then?

I have a bi suitor and she was really into threesome, said it's really fun and wild. I'm not into it though, not my cup of tea, I don't know what approach one should use, I have no experience in this and I don't think I will in the future.

See my hubby and I want to do a threesome but I do not want to do it with someone I know, I want to go downtown and meet someone random, that way I won't feel uncomfortable afterwards seeing them around.
