Personification of death where did it came from?

Well im wondering about this one, the skeleton man with a black cape and a scythe with just one finger can cause you death. I have heard somewhere that it is from a story but can't seem to find this one. Well you can see this kind of personification in book, cartoons, movies, video games and such but i can't seem to figure out how or where it started. do anyone has an answer to this one.
