Help! I need a formal written to report a nurse to DON how she treated me!

A nurse who had asked me to help her put the patient into the commode so that she can get a urine sample. She left the room and command me to get out the room also for patient privacy. later, I came back within 10 minutes to check the patient. I noticed the patient is laying on the floor and the commode was upside down. Urine is all over the floor. I quickly reported the incident to the nurse. This is how I said words to words, "Jane, he is on the floor, and the commode is upside down" Jane replied, "Who, who is he?" I said "The patient,Mr JOHN DOE!" and she replied with harsh voice, "Can't you speak english?, Can't you understand english? You always confused me" What did I do wrong in this scenario? Did she said such words just because I am minority? Now wait a minute, If she is talking like these! Isn't it possible also that she can treat any patients the same? I mean not treating them right? To me the way she treated me was a verbal abuse, If she has the ability to treat me like that, Isn't it also possible that she can verbally abuse the patient also? She is an LPN. These has to stop! My DON knows the concern and had ask me to write a report. Please would anyone make a rough draft for me and send it to my inbox? Thanks and I appreciate all you people who will help me in this situation. This really bothers me so much.

first off is there a special form you have to use? If not what I would do is type it up just the way you did with your post. Explaining the whole situation from Start to finish. Show it to your DON and if they ask for more. ask for help from the DON they should be able to help you. I myself work with a lot of different nationalities and sometimes understanding them is hard I just usually ask it they repeat them selves just so I know I got everything right. Good Luck

The thing is I clearly says it! Not too fast and not too slow. Basically, this nurse has a problem with color coded human. She is no better than anyone in this world.
