when I add coolant and/or water with the car running I can see the water moving in its container. Then when I drive a short distance I need to add water again. there is no obvious leakage so where is the water going? can the problem be a bad rad relay fuse? or just change the thermostat

I would check your thermostat. Unless your radiator has gone bad, your thermostat should be next to check. Good luck.
