A patient was placed on a medication and it worked, but a few years later, it started working the other way. What does "working the other way" mean?

In the context you provided, my best guess is that it means that the medication that he is taking is now having the opposite effect for what they were intended for. I have heard that some medications meant to alleviate a problem have side effects that may actually worsen it. Perhaps this patient is taking the medicine and is now suffering side effects.

i have gussed the same.. the person may be suffering from few side effects and if he had taken the medicine it might be possible that it is not helping him the same way as it had in the past
