smart card:create file apdu(ins=eo) command format

Can anyone kindly explain me how to form the APDU command for creating a file in Smart card(MF,DF or EF). I have searched many websites but nowhere I could find the exact answer I am looking for. I will also be a great help if someone could share ISO 7816-9 document with as I could not find the full version of this document in the net. Thank you all for contributing your knowledge on smart card.
