I'm sure you have tried everything but I have a few suggestions that helped my grandson. We bought him "big boy" underwear and he didn't want to get them dirty. We also got a potty seat that sits right on the big potty and a potty chair that we keep right out in the living room. I had trouble with my son too. He was about four when I got finally got him trained.

He's too scared even with the thing on top the toilet.

have you asked the doctor about him being emotional senstitive? sometimes kids that are emotionally sensetive get stressed out about simple things. My younger sister was like this and and when she was like 2 she would get stressed out because the toe line on her socks did not line up with her toes, and she got stressed out because when her shoes were tied the hoops on her laces were uneven. Simple stuff like that. Your son may be like this! I think he wil lgrow out of it eventually but you have to keep trying!
