When an insured/patient/Claimant files a claim, not lawsuit, juts claim for benefits, is a medical authorization necessary to obtain records from the treating physician?

Since medical records are considered private information, the claimant would have to sign a release allowing the Dr to release their medical records. As far as I know, the only party allowed to view the medical records without authorization would be the insurance company. This is due to liability issues. Many Hospitals and Docotrs have gotten into legal trouble disclosing a pateints medical records.
