What is the worse service you've gotten in a restaurant?

We all go out to eat from time to time. Every now and then you hit a place where your service isn't up to par. What happened, did you complain to the manager, boycott the place?

A couple of weeks ago, we waited 15 minutes for a menu and water and actually had to get our own menus. We waited another 40 minutes for our appetizer which was chips and bean dip that came out dry and hard. We sent it back and after another 15 minutes, we left.
