Weaning from the bottle - help!

My 14 month old son has a very bad habit of sleeping with his bottle. I have tried four times to wean him off night bottles, but it has never worked. Sometimes I just go nuts listening to him cry all day and night, sometimes his dad gives him a bottle and destroys all my hard work. He wakes up a minimum of 5-6 times a night, often more and is soaked through a couple of times. Anyone have good ideas on how to wean him off the bottle without going completely insane?
