I am in a similar predicament. Apparently I was prescribed too low of a dose the first time I was on Accutane, so I was advised to go through another 5-month course. My biggest dilemma is getting through all the bureaucratic red tape, i.e., the iPledge program. Of course the choice is up to you. Isotretinoin is an extremely powerful drug and can damage liver function if taken repeatedly - so I wouldn't recommend taking it again unless your acne is not responding to topical creams/cleansers and antibiotics.
