| | If she needs Ensure or one of their products to drink, here is what was sent to me. I downloaded the form and filled out the information. I attached a copy of her medicare card. I went to the Mecicare site and found a section that states that medicare does NOT pay for nutritional drinks and got a copy from DHR saying her secondary insurance does not cover them either. I attached proof of her SS income. Her doctor signed the form and I mailed it to the company. They mailed the cans of Ensure directly to us

you can click on the link titled "View a list of participating pharmaceutical companies." From there, please select "Ross Medical Nutritionals Patient Assistance Program" that is referenced under Abbott Laboratories.
