How to write a letter to a school asking for the Birth Certificate

My sis is going to a new school since September 2009.
today my mum went to the OLD SCHOOL to get the Birth Certificate. They have asked us to give them letter saying , we want to get the child from that school as we are sending her to another school and in regards to that we want to collect the original Birth Certificate...
I am not good in writing formal letters...
Please help me...
My sis name is Harishma . She studied in the old school still Grade 6

Does schools issue birth certificates? generally they are issued by the health and statistical departments, If schools are issuing authorities in your place, then here is the format.

Respected Sir/Madam,
I the undersigned is the parent of Harishma of Grade 6, herewith request you to issue me a Birth Certificate of my ward, to enable me, readmit her to a new school, located close to our residence.
Please do the needful and oblige.

Thanks for the format.
It's not that they issue Birth Certificate.
We parents have to submit an original Birth Certificate in the school in which we want to admit our kids and the time the child leaves that school, the birth certificate will given back to the parent
Most school ask for the Birth Certificate.
But the the newly admitted school, they didn't ask for and birth certificates. so we also didn't bother much...
Now only my mum went to my Sister's OLD SCHOOL to ask for the certificate and they have asked to submit a letter asking why we need to the Birth Certificate...

It is not a difficult task to write a letter asking for the Birth Certificate.
Here are some tips you can follow:
At first, make sure the letter is to the point and shortly described about your request or you may be refused. At the same time, give the detail about your sister, such as date of birth and signature and so on.
When you have finished it, remember to type the letter and avoid hand written applications or letters. At last, give them time to reply your letter and process on your application your letter will take time to process.
