First they deprive him of his heart meds and substitute one of David's concoctions and then put him under a lot of stress and then they can't understand why he had a heart attack. They have been blaming Annie for all kinds of things while they have been kidnapping and torturing people, breaking people out of jail, attempting to kill people which started the whole murder storyline, neglecting children(Ryan by leaving her at the mansion on the night of the murder because he had to care for Kendall and by letting her ride her bike home from school all alone), locking people in elevators, etc. Only by Annie appearing as an angel did Adam survive.
