What format of a formal letter must I use?

I have been absent from school for more than a month already due to on and off sickness. It's been a pain and my mother is worried so she went to my school and talked to my dean. She was told to create a letter addressed to him that will request for extra assignments/project and work from my professors that I must complete in order to at least get a grade and not fail all my subjects.

Your home address top right of the letter. Date of the letter below your address.
The Deans address top left of the sheet.

Dear Sir,
Unfortunatly my daughter has been to ill to attend classes for the last six weeks and our doctors advise that they anticipate that it will be another xx weeks before a recovery is made sufficient to return to school. Copies of our doctors reports attached.
It would be appreciated if her professors would kindly supply sufficient assignments/project work to allow her to continue her studies and any other assistance to help her at this critical period of her education would be much appreciated.

Thank you very much, smacksman! You just gave me everything I needed.
Heaven only knows what I will type in my computer to create the letter while nursing a very painful headache. I might make a diary that says "it's so painful" instead.
I am still far from allowed to go to school according to my mom with the ending statement "unless if you wish to pass them your virus that's sitting on the sides of your neck and getting yourself another week of fever, be my guest."

wow smacksman, that is a very elaborate and fully detailed letter you made there, i guess in this letter alone you can get an A sinful rose, but anyway tak care of yourself lot's of rest, fluids and try to rad on some of the subjects that youve already bypassed, get some from your friends, this is great smacksman.

The letter format described in the first comment is perfect for you Mother to follow. What I would advise is also have the doctor hand you a letter too to attach to your Mothers before handing it in to the dean of the school. It may be a requirement or may not but you are covering all the bases clarifying why you were absent and that you were truly sick. The doctor can't reveal specifics but can state that you were under his care and to make a notation that you fell sick and was out of school from this date and are now able to return to school as of this date. The two letter together should allow you to return and get an extension on the classes, test or labs that you missed out on.

Thanks anyway for taking the time to explain the situation. Sorry to hear that this affects you, your family and the whole area you live in. That has to be hard but it is good to hear that you are doing everything you can to improve your health.
Also glad to hear that the dean is aware and is willing to help you out with your schooling.
Good luck with your letter and I hope you get the time and ability to make up the work you missed while being out sick.

Well still in here people are not following the left side rule i mean where everthing is inleft including the signature.
Suggest you to first write your adress at top ( which is leave blank didn't write for schools even now in my coll i didn't) Then "To the dean or principal with the school name then respected then for the sub or so called refernce for thereason. Then the contents which must not have more than two paragraphs. At end post you signsture.
