"Those who are well do not need a physician, but the sick do." Superficially, it is fine to ignore someone if he or she is fine. The writing on the signboard only means that don't pretend to be sick if you are well and don't bother the doctor unnecessarily.

You have a point. Some people can pretend sick just to take advantage of the kindness of others. Perhaps they also want to feel sympathy selfishly.

Usually a sick person needs to visit a doctor. A healthy person if he goes to a doctor for a check up then the doctor will think there is something wrong with this person's mind.

In real life, some patients are malingering in nature. Pretending that they are sick or too self-conscious to be healthy.
