I do not smoke

It is a disgusting stinky habit. I also have asthma. No thank you. I was surprised to discover a page in my medical records stating that I was counseled about the effects of smoking with my medical conditions. It also said that I showed up in the ER with a nicotine patch and was given them during my stay in the hospital. WHAT?!!?!? I was given all sorts of medications and supplements while I was hospitalized but nicotine patches were not included. It is kind of scary to see that incorrect information in my medical file.

That does not make any sense, but how are you going to have them admit that it is not true?

They can say that I smoke. I don't. It isn't illegal. I don't have any lung damage than from asthma. I don't think there is any way to refute what they put in there.

When my husband was discharged from the hospital, he'd been given two other people's discharge instructions along with his own. I understand hospitals see thousands upon thousands of patients, but seriously? You would think they would keep their records streight. But, some reason or another.... they can't. Hell, I had to cancel an appoitment sent to him for sept 1, in Gamville because we're not driving that far nor did he make an appoitment to see that department. Any medical would be done either at the VA clinic or local hospital.

Dang I hope this isn't long. My wife had a surgery in 1984. TMJ implants that failed. Dow Corning was ruled responsible for damages. It took ten years of legal action. Beside the point, sorry. When records started to go electronic, mistakes would come up. She is a non smoker too but records said she was a heavy smoker. We requested copies of medical records from everyone she went to. We kept copies and sent the rest of them to the attorney representing her vs. Dow. Dow tried to get the physical copies ruled inadmissable because their electronic copies were so different. We sat down with the attorney and reviewed the electronic records and it made me sick to read what was included. It was as if they mixed up three or more people and added it all to her records. We got the electronic records booted and Dow settled. Ever since then, after either of us has an appointment, we request a physical copy of the record for that visit and call to correct any inaccuracies. People make mistakes. I just always cover my bases now to avoid the surprise and angst you must have felt when you saw your records. Call and correct the information and request a physical copy with the corrected records. Do this for everyone and you have a safety. That wasn't that long, was it?

Thank you for the response. I will definitely ask for hard copies in the future. I guess I am just too trusting. I don't even take my copy of receipts from restaurants/bars.

@NJChicaa I was too trusting too. Now I'm not lol. I keep every recipt and use them at tax time to increase my taxes paid and lower my liability, giving me a larger refund. I hope you get it straightened out right away. That information is available to insurance companies. Your rates are based in part on your medical history. Good luck NJ.
