Appealing My Health Insurance Claim Denial

I am seeing a health care provider that doesn't take insurance / doesn't file insurance. I have to pay her in full at the time of service and submit my own claim form to my health insurance company. They initially denied my claim because it didn't have all the information on the bill that they wanted, like ICD9 Diagnosis Code, CPT Code, etc. So I have created a spreadsheet with all the information they should need and written a Letter of Appeal along with copies of my cancelled checks providing payment and hopefully I'll get some reimbursement. This is my first time having a doctor that didn't take insurance and it's a pain but she's really good and I want to keep seeing her.

I would have left as soon as they said they didn't take/file insurance claims. That's their job, not mine!
