1994 Chrysler Lebaron Serpentine Belt keeps coming off and/or breaks

I have a 1994 Chrysler Lebaron and I've had to replace the serpenting belt 3 times in one year. It has come off a few times and we just put it back on. Have any idea why this keeps happening? After looking through the Auto Repair...
