Nurses Vs Case Managers

I have nothing wrong with either job title let me just preface with that. I get it and I get that they may have to give you news that you just don't want to hear. Get it, got it, fine.
We have been talking to dad's nurses twice a day every day since he's been in this long term facility for his vent.
He has gotten good reviews and everyone says that he is on track. he will be there longer than 30 days just because we really want him to be ok and not land back in there or the hospital. Rehab is not part of the plan this time.
Case manager says it doesn't look good. He probably won't come off the vent and we need to think of a longer term facility.
Where is the disconnect here?
Mom just got up there and she called me when the nurse was in there. She looked at my dad, his vitals, his notes, no concerns.
We're making progress just not fast progress which we knew because of his age and everything that was wrong.
I don't get it. Unless he's trying to get us to send dad to the other facility that he's giving fake info which I don't want to believe but I mean it could happen.
Respiratory therapy is being asked to come in and I'll find out more then but honestly I don't get it.

There's usually a delay or communication breakdown between the two sides of the job. Nurses know more, because they see him everyday. Case workers might check in every now and then, and often get their info directly from the doctors under his care. In some cases, the doctor's info might not be as up-to-date as you'd like, because nurses typically update the patient information at the start and end of their shift, unless something urgent comes along.
I have to wonder, if vent care and training is an option for your family so he can be managed at home? Or is he too sick for that?

It's not that he's too sick, the goal is to get him off of the trach and we're headed in that direction. The problem became the facility he's at is only a 30 day facility. They never told us that so when they started trying to force rehab that's why. We got a call from a VA rep and he may just stay in Nashville. It's another 30 day facility but I think that will be fine if he keeps going the way he is.

@kaylachan We're hoping. No one told us before we left the Cookeville Hospital or we would have tried to find something else. Everything seems to go well for a while then the shoe drops so we just take it day by day.

These two are evaluating things from completely different perspectives. The case manager deals with the facts. The nurse is there to make everyone feel better. A second opinion is always good.

@JordanLader ........Interesting. This one facility he is in now seems like a waste of time and money if he will need aid more than 30 days down the line. I don't understand how the health industry works sometimes.

@celticeagle I don't either. No one ever told us it was only 30 days at least the VA rep said up front that this next one is the same. With what I'm seeing though I think that will be more than enough time. He's already doing 12 hours off the vent, next is the swallow test and 14 hours.

Nurses are trained not to give hope but what they see. At least they are down here. Same with first responders. We can't say things are good if they're not and we can't say well, looks like the end, at all. I've never had a nurse give me false hope, I've had a dr give me doom and gloom though where it wasn't needed.
It turns out we just weren't disclosed everything. Select is a 30 day only facility. So once his 30 days are up he'll need moved and with him still on the vent it'll have to be another facility. It just doesn't have to be in Kentucky.

It really is. Turns out we just weren't disclosed everything. Select is a 30 day facility. After 30 days he needs to go somewhere else or home. With the vent he has to go to another facility but it doesn't have to be in Kentucky.

It is. The disconnect is bigger than I ever saw in a hospital. However we did figure it out. Select is a 30 day facility so after 30 days he has to go somewhere else since he is still on the vent.

I ran into the same thing with my hubby. They said he was doing so well; but when he tried to come home for a day he fell three times. Ask a lot of questions and insist on talking to someone in authority.

We did and we found out that the place he's at is only a 30 day facility which no one told us. So they wanted to transfer him to another facility.
