Experts in psychology have been watching Trump's behavior, some for decades. Early on he was diagnosed with malignant narcissism and more recently frontal temporal dementia. These two run separately and do not usually coexist. Instances of both are rather low in the population.
The strange things he chooses to do and the chaos they cause tend to compound eachother. He has to dominate in all areas and be in the limelight all of the time. His dementia causes misuse of words, confusion, and reciting untrue stories number over his need to always be right. What a he'll that must be.
As far as his decline progression goes it may vary. He has the best Healthcare in the country so he may hold on for a time. But escalation is probably in the future. He will have a need to do more of it and for it to be bigger.

Oh how I wish they would get that evil and crazy B. out of office. Somebody needs to come with a plan, and in a hurry before he blows everything up literally..I heard at one point he was reaching for the nuclear codes.

They get worse as time progresses, forgetting things is the least problem of dementia. The brain seems to get violent and why dementia patients so scary and say mean things. Why that Kansas City Chiefs player who killed his family them him self said study his brain and made a point not to shoot himself in the head so they could study his brain.
