Choosing A Book Cover

I have been struggling to put together a book cover for my historical suspense romance book. It turns out that I have a picture in my mind and cannot find anything I like that measures out to it.
So, I have decided to start with the background and work my way out, the way you do when you paint a picture.
What you see in my picture here is the background. I want to add a little something to the foreground but have not yet decided what.
I know it needs to be simple and uncluttered so I will keep mull it over in my mind.
Who new it would be this difficult for me to make a decision.

Good luck, I know how hard it is to see something in your mind's eye and then it doesn't come out on paper
