How are you?

It's commonly taught in non-English speaking countries that the best response to how are you is "I'm fine thank you. And you?"
It's something that I don't usually use when a friend asks me that. I usually say I'm ok.
However, don't always believe that when someone says "I'm ok" it means they truly are. Coz most of the time, behind those words, is someone who's in a not so good mood.
Are you ok?

And that is true. If I tell them I'm having some issues would they be able to help? What would they really do if I tell them the truth? That's what comes to my mind.
And another reason why I say I'm ok is that I don't want to explain further what I'm truly feeling. Because if I say I'm not, then the conversation would never end.

@toniganzon - Exactly, anyway they cannot help and if you say I am not well they want to know what happens and I do not like to discuss personal matters.

I think it’s just a standard response we all use just to make small talk, without getting into the specific details of our lives. I know I will say it all the time, even if I’m not okay. But usually I will open up more around people that I am closest to.
