I am quite mixed up

I thought I had an appointment today. I was busy filling out the paperwork, and dating everything 8/15, today's date.
I looked at one of the pages I had to sign, with the privacy notices. Turns out, my appointment is 9/15. I had entered it wrong on my master calendar.
I thought I was wrong, about it being this month, but they sent me the paperwork before the first of this month. I was confused.

I would, too. I had that happen to us, too. Last year we were sent new patient payerwork a month before the appoitment.

Pretty did that a couple of weeks ago. She thought she had a doctor appointment and, since I couldn't see, she walked to our doctor's office and then came back about half an hour later and asked me why I didn't tell her she was off on her day/date. As if I pay attention to the day/date when I don't have any appointments. 
At least all the paperwork is filled out now.

I once messed up a dentist appointment because I thought it was at 2:00 on the 10th. It turns out, it was at 10:00 on the 2nd! I had accidentally transposed the numbers when I wrote it down on my calendar!

At;least the date was not earlier in which case you would have missed the appointment

I write everything down as soon as I know about it but it still doesn't always stop me getting confused.

That is a bit confusing sending the paperwork ahead a month early. At least you have it taken care of.
